CDC Logistics, L.C. requires the following information to be filled out and faxed to our office.  These files are in PDF format.  You can simply click on the document file name and print out the form or save it on your computer:

We will also need the following information:

1)   Copy of your Federal Operating Authority showing your MC number and legal company name.

2)   Copy of current insurance certificate with $50,000 cargo and $750,000 auto liability coverage.
      Please note that CDC Logistics, L.C. must be listed on the certificate as the "Certificate Holder".

Once we receive the above information, we will issue you an account number which you will reference on all future correspondence.  When you receive the contract back from CDC Logistics, L.C. you will be added to our approved "Carrier List".
